    • Application Mod.RP5002-DGSS
    • Identification document of the applicant and spouse
    • Income declaration for IRS purposes, in case the applicant and his/her spouse are legally obliged to present it
    • Documents proving income, in case the applicant and his/her spouse are not required to submit an IRS declaration
    • IBAN proof, in case you want the payment to be made by bank transfer
    • Document proving the value of the real estate assets
    • Legal residence title, in case of refugees or stateless persons
    • Identification Form for Individuals Covered by the Social Protection System of Citizenship (Mod. RV 1017-DGSS), in case you are not registered in Social Security, together with the requested documents
    • Declaration of Disability Situation Caused by Intervention of Third Parties (Mod. RP 5074-DGSS), if applicable.
